Assisted Living

An assisted living residence or assisted living facility is a housing facility for people with disabilities or for adults who can still live independently but who need some assistance. Depending on the facility, that assistance may include help with meal preparation, housekeeping, medication management, bathing, dressing, transportation, and some nursing care. 

Independent Living Arrangements

Community Resources & Services

Many community members are unable to follow their medical care plans because of non-medical barriers such as lack of access to transportation, nutritious meals, or affordable medications. Our program was developed to promote healthy behaviors while helping community members access essential needs.

We connect people with all of the following services:

  • Service coordination (help with managing care needs and services)

  • Personal support (assistance with daily living activities, i.e. bathing, dressing, meals and housekeeping)

  • Home health services (nursing, home health aide, and occupational, physical and speech therapy)

  • Emergency response systems

  • Respite care (caregiver relief)
